All observations and documentation are to be made on this form. Rlskman Is used for capturing and auditing. all "Rapid ResponseD and ~~code Blue events. This enables Identification of clinical variables which may affect outcomes and identify areas for improvement. It is an expect~tion that post any Rapid Response or Code Blue a Riskman is completed by the Prima,Y Nurse or Team Leader.
