Review into DDYDC - Kendrick 21 April 2017 4 BACKGROUND On 5 April 2017, two detainees of the DDYDC escaped through a window from the low security area (S Block). The detainees were at large for two and a half days before being apprehended separately on 8 April 2017. The detainees were: Josiah Binsaris, aged 17 years. Came into DDYDC as a remand detainee on 14 March 2017 for a charge of Drive Vehicle in a Manner Dangerous. Treh Mawson, aged 17 years. Came into DDYDC as a remand detainee on 9 March 2017 for a charge of Aggravated Assault. The review also audited security with a view to: understanding the decisions and events that led to the security breach on 5 April; and identifying where action is needed to avoid future security breaches including across infrastructure and operational directives, to ensure the safety of employees, detainees and the public. METHODOLOGY The Review was conducted between 10 April 2017 and 13 April 2017. The methodology included: site visits to the DDYDC including S Block, escape area and the Control Room; interviews with staff at the DDYDC, staff from Territory Families (the department) and the CEO Danila Dilba Health Service; review of documentation, policies, incident reports and other correspondence relevant to the investigation; attending a Post Incident Debrief undertaken by the Departments Deputy CEO on 11 April 2017; and compilation of a draft report and recommendations and review of such with Departmental Executive staff. submission of final report by close of business on 13 April 2017. RECOMMENDATIONS Recommendations arising from the review appear as they arise within the content of this Report and collectively at the end of the report from page 17.
