PROGRESS OF THE INVESTIGATION 3. The Investigation was commenced in 2007, however due to the Commissioners limited resources, the finalisation of the Investigation and Report became significantly delayed. On 22 July 2010 the then Commissioner authorised Ms Raelene Webb QC, assisted by Ms Elizabeth Armitage, to complete the Investigation pursuant to section 50(1) of the Act. The Investigation was progressed following the appointment of the current Commissioner in September 2010. 4. In January 2012 comments were sought from relevant parties on factual aspects of the report only. Following further investigation a complete draft of the report was circulated for comment to respondent organisations in December 2012. Responses were received from the Department of Health and Office of the Public Guardian and further consultation with NAAJA occurred between April and May 2013. The final report on the Investigation was provided to parties on 27 June 2013. 5. The Commissioner acknowledges the significant delay in finalising this Investigation and on behalf of the HCSCC apologises to the parties
