We require an experienced, qualified and motivated person to help us to build on recent initiatives in primary health care aimed at improving the health of local people in Aboriginal communities on the Gove Peninsula. The position involves working in a clinical framework for acute care with team members to implement various initiatives under the primary health care program. You will also be working with others on strategies aimed at improving the level of awareness of chronic disease in the community and improving the participation rate of those Aboriginal people affected by chronic disease, in obtaining regular health services. The minimum requirement for the position is a qualification as an RN4. In addition there should be a commitment to working in a team situation with Aboriginal people and an ability to communicate effectively with team members and clients. TEAMhealths services are provided within a model of recovery focussed psychosocial rehabilitation and Within the framework of the National Mental Health Plan (1996) and Standards. TEAMhealth works closely with other specialist service providers such as Government Mental Health Services, drug/alcohol services and non-government organisations. The Sub Acute Program provides intensive short term recovery focussed psychosocial rehabilition to people with mental illness and psychiatric disability. The goal of the program is to prevent admission to, or facilitate early discharge from, the psychiatric ward in Royal Darwin Hospital. The PRW is responsible for the preparation and implmentation of individual support plans in consultation with the Residential Program Manager. Relevant experience and/or qualifications in a health discipline will be highly regarded for this position.
