This project is looking at renal care coordination across the NT for patients with significant renal disease who do not yet require dialysis. A Barrier Analysis and Patient Journey Modelling was done which highlighted issues such as the complexity of the journey for CKD patients, multiple silos of information and computer systems, and lack of formal referral processes and documentation. The aim of the project is to reduce duplication of work, improve information and communication across the different services and allow more patient friendly service provision as close to their home as possible. The project is now implementing some strategies to address those issues through care coordination of patients provided by public health nurses (14 at DHF and 4 employed by the large, regional AMSs in Darwin, Katherine, Tennant Creek and Alice Springs) using Quality Improvement cycles.
