John Murray Forman, also known as Jack, was born in 1909, Perth, Western Australia, to Alfred A. and Louisa Forman, of Mosman, New South Wales.
He is remembered by his family and Captain Bill and A. Robertson in newspaper notices; Forman is also commemorated at The Northern Territory Memorial, which stands in the Adelaide River War Cemetery.,
History
He joined the Merchant Navy in Cremorne; the Commemorative Roll indicates that Forman was the 2nd Officer on the Neptuna. He died at the age of thirty-two on the Neptuna in Darwin on the 19 February 1942.,
